1. Initial Contact: Once you reach out to Unity, our service manager will get in touch to arrange a convenient time to visit you at home. During this visit, we will assess your individual support needs. Feel free to have friends or family present during any of these visits.

  3. Discussion and Planning: We will discuss the type of support you need and how we can best provide it based on your assessment. We’ll also talk about suitable days and times for the support, and introduce you to the support worker who will be assisting you. This is also a great time to ask any questions you might have.

  5. Tailored Support Plan: After the initial assessment, we will create a personalised support plan tailored to your needs, including any necessary risk assessments to ensure your support is provided safely.

  7. Review and Agreement: The service manager will visit you again with your allocated support worker to review the support plan with you. If you’re happy with the plan and any risk assessments, we’ll ask you to sign them to confirm your agreement. You’ll also sign a service contract before your support begins.

  9. Client Pack: A client pack will be kept in your home, containing a copy of your support plan, risk assessments, your contract, and other useful information related to our service.

Personal Care services

Unity is registered with the CQC and is able to deliver personal care if required. Personal care will only be delivered if it is discussed in advance during the initial assessment and included in the client care and support plan.
 

Visits

Visits can be scheduled on a weekly, fortnightly or monthly basis. We don’t usually provide more that 2 visits in a week.

Please note: Our working hours are 9am to 4pm Monday to Friday.

We do not provide support on weekends, bank holidays or out of normal office hours.
 

Pricing

Take a break Service

Our Take a Break Service is charged at £20 per hour with a minimum of 2 hour visit time.

Personal Care services under the Take a Break service will be charged at an additional £2.50ph
 

Home Support

Our Home Support service is charged at £22.50 per hour with a minimum of 1 hour visit time.

Mileage

We charge for mileage incurred during a visit, not for travel to and from a client’s home.

This is charged at 45p per mile and will be included on your monthly invoice.
 

Invoicing

You will be invoiced in monthly in arrears for the hours delivered. We ask for a minimum of 24 hours notice if you wish to cancel a visit. If your visit is cancelled with less that 24 hours notice you will still be charged for that visit, unless there are exceptional circumstances.
