Role Summary

To work in the community with individuals aged 65 and over who would benefit from short-term low-level support to help them remain independent in their own homes and reduce where possible reliance on Adult Services and NHS services. This post is a 1-year initial contract with the possibility of extending subject to funding.

Responsibilities:

  • To enable independence wherever possible and provide encouragement and motivation with the aim to avoid dependency or a worsening of current circumstances.
  • To promote individuality, choice, independence, dignity, respect for the individual and partnership working
  • To enable individuals to identify personal goals.
  • Ensure vulnerable people are safeguarded from abuse and to report any instances of alleged abuse which you witness or become aware of
  • To provide Home Support to Unity clients using a person-centred approach and in the least intrusive way to include:
  • Identifying social support schemes of interest to the client and then linking individuals to community-based resources to reduce social isolation, including befriending services, social clubs, better balance classes etc.
  • ‘Warm handover’ to organisations as appropriate (including attending initial visits to build confidence)
  • Reducing the risk of food and fuel poverty
  • Enabling individuals to use assistive technology e.g. automated medication systems
  • Enabling individuals to improve their home environment.
  • Enabling individuals to develop ways of managing their finances/ post/ appointments and maximising access to financial assistance.
  • Supporting clients with applying for benefits and financial assessments
  • Making phone calls on behalf of the client.
  • Assisting clients to appointments
  • Building relationships with local communities
  • To work with other agencies as required.
  • To support people who display hoarding behaviours, being empathetic to their needs and show compassion and patience.
  • To keep good records, communicate effectively and share information as appropriate whilst maintaining confidentiality and data protection requirements in line with Unity policies
  • To deliver other projects as appropriate
  • To produce statistics, paperwork and case studies as required
  • To be an effective team member, demonstrating flexibility and resourcefulness, and co-operate with outside agencies.
  • To attend staff meetings and other events as required
  • To provide holiday cover where necessary
  • To undertake relevant training as required
  • To complete the mandatory induction (including training) within the first 12 weeks of employment
  • To adhere to quality standards
  • To comply with all Unity policies

 

Essential Skills/Experience

  • Experience Community Development or Health & Social Care or equivalent
  • ICT skills/Microsoft office/internet/email
  • Experience in working with vulnerable people
  • Experienced in problem solving
  • Ability to meet tight deadlines and to achieve targets
  • Ability to monitor and evaluate services and produce reports and statistics as required
  • Strong interpersonal skills and an ability to establish a good working relationship with a range of people
  • Ability to work in partnership with various agencies
  • Able to work on own initiative and be part of a team
  • Full, clean driving licence

 

Desirable Skills / Experience

Knowledge of the Voluntary Sector

Available to work additional hours as required and to work some unsocial hours
