How our Health & Care processes work

How does the process work?

Once you have contacted Unity, the service manager will contact you and arrange a convenient time to visit you at home to complete an assessment of your individual support needs. You are welcome to have any friends or family present during any of the visits.

We will discuss the type of support you would like Unity to provide, and how we can best deliver the support based on your individual assessment. We will also discuss suitable days and times, and the support worker who will be supporting you. It will also be an opportunity to answer any questions you may have.

After the initial assessment, we will draw up a support plan tailored to your individual support needs, and any risk assessments that may be needed to provide your support safely.

The service manager will then visit you again with your allocated support worker to go through your support plan with you. If you are happy with the plan and any accompanying risk assessments, we will ask you to sign them to say you are in agreement with the plan. We will also ask you to sign a service contract before your support commences.

A client pack will be kept in your house containing a copy of your support plan, risk assessments, your contract and other useful information relating to the delivery of our service.

Personal Care services

Unity is registered with the CQC and is able to deliver personal care if required. Personal care will only be delivered if it is discussed in advance during the initial assessment and included in the client care and support plan.

Visits

Visits can be scheduled on a weekly, fortnightly or monthly basis. We don’t usually provide more that 2 visits in a week.

Please note: Our working hours are 9am to 4pm Monday to Friday.

We do not provide support on weekends, bank holidays or out of normal office hours.

Pricing

Take a break Service

Our Take a Break Service is charged at £20 per hour with a minimum of 2 hour visit time.

Personal Care services under the Take a Break service will be charged at an additional £2.50ph

Home Support

Our Home Support service is charged at £22.50 per hour with a minimum of 1 hour visit time.

Prices valid from November 2023

Mileage

We charge for mileage incurred during a visit, not for travel to and from a client’s home.

This is charged at 45p per mile and will be included on your monthly invoice.

Invoicing

You will be invoiced in monthly in arrears for the hours delivered. We ask for a minimum of 24 hours notice if you wish to cancel a visit. If your visit is cancelled with less that 24 hours notice you will still be charged for that visit, unless there are exceptional circumstances.
